To submit a Service or Damage Request,

1. Go to your order tracking page, by going to https://www.ajmadison.com/customer/order_track/
Enter your order number and email address

2. Under each item there will be a blue button that will alow you to submit your damage report

3. We will respond within 24 - 48 hours.
